Default aetna runaround

I have Aetna POS, So far they have lost my pre-determination paperwork twice, have told my doctors office they dont need a pre-authorization code, that its covered and that my place of employment has "opted out of pre-authorization" which my employer's HR department has no idea what they are talking about, and now our Aetna rep told my HR person today "I am trying to get this pushed through as an urgent. The provider did send this pre-d to us on 11/7/08 but usually the turn around time is 30 days."
So my surgery is scheduled for Tuesday, today if Friday, as of yesterday the surgery center had no record of me on their schedule and I don't know if the insurance is approved.
And I have to leave to go out of town for the weekend for work in 15 minutes. I'm very stressed. I don't want to go through this mental preparation again if we have to re-schedule. Not to mention all the stuff to get ready to be out of work and out of commission at home.

1988:sprained neck roller skating
1989:dislocated neck in a freak accident
1993:MVA
1995:diagnosed with bulging disc C5/6
2000:disk herniated
2000-2004:multiple treatments including nerve root block, trigger point injections, epidural steroid injections, pt
2005-2008: pain meds and muscle relaxers only
ProDisc-C replacement on c5/c6 11/18/08
