Anyone have Unicare?

I have a Unicare self-employed policy issued in Texas. I just recieved a non-certification letter for the pro-disc L4/5 surgery, stating it was experimental. Has anyone have Unicare pay for this? What do I need to include in an appeal? The doctor is writing a letter for my appeal. Has anyone included studies and got approval that way? Any help at all would be HUGELY appreciated.
