Insurance Appeals Letters

A long time ago, I seem to recall their being some examples of letters people used to get their insurance comapanies to pay. I have tried searching again, and perhaps its just the giddiness of reading that people successfully received reimbursement for overseas surgery, but I can't seem to find them. I'd obviously be interested in ones specifically for overseas reimbursement, but any successful letters would be a great help. I'm going to look into kickstarting this process by the end of the week!
