In need of insurance precedents

I'm six days post op with a two level Prodisc C. Feeling really good but now I have the task of fighting Anthem Blue Cross for the reimbursement of the surgery. Part of my appeal is to develop an insurance precedent, which would be a list of those whom have successfully appealed their insurance company or had initial approval. It would be greatly appreciated if you could please PM me the following for my appeal:

Date of surgery
Type of surgery
Name of hospital
Name of doctor
Name of insurance company
