Default Need Help-For those of you who have had multi level Cervical ADR Approvals

Please email me. Id like to get a copy of your written approval from your insurance co so I can use it in my appeals with Aetna for proving prescendence. Pls make sure it has your name, doctor's name, and the adr procedure listed on it.

Some of you may have been thorugh the appeals process before and may have a stack of other peoples approvals I can use also.

4/08- DDD at C5/6 & C6/7 & bulging discs. C5/6 portrusion.

6/08- Disco results- C6/7 painful, C5/6 popping sounds

7/08- Plasma disc decompression-significant relief obtained

11/08- pain returned to almost pre surgical levels

1/09 -Disco w/ Dr Ziglar shows C5/6 & C6/7 painful-2 level ADR recommended

2/26/09 - c4-c7 ADR Prodisc Nova with Dr Bertagnoli. 100% Success but need C6/7
ADR revision due to subsidence.

I am trying to get two level (C5/C6-C6/C7) Prodisc - C covered in North Carolina, and would appreciate any approvals for multi-level cervical ADR from anyone anywhere. Please email to jmichaeldail@gmail.com.
