c 4/5 5/6 peek disc replacement 7/2007 failed fusion c 4/5 5/6 revision 9/2008 titanium plate and screws replaced
c 3/4 stand alone peek cage 10/2009
demerol robaxin aleve daily
sever ddd at c 6/7 t1 ddd c 2/3 bone spurs
facet joint injections c3-7 Dec 2010
rhizotomy c6/7 only Jan 2011 failed
