It's almost good that you've had to wait so long for treatment. It worked in a similar way for me. If I had gone ahead with surgery when I first started to appeal with my insurance, I wouldn't have known about my severe metal allergies.
They eventually led me to Brazil with Dr. Luiz Pimenta, one of the very best surgeons in the world, but not well known here in North America. He was wonderful to work with, and spent a lot of time answering our numerous questions.
My price for three levels was $35,000 US, which included as much time in the hospital as needed.
One thing to think about with your search is the device (ADR) that each surgeon uses. Dr. Bertagnoli uses ProDisc almost exclusively, while others go with the M6, which, if you look at many of the posts online, seems to have fewer problems. There seem to be a lot of people who are unhappy with the ProDisc for whatever reason.
So both the surgeon and the device are extremely important. Not to throw a wrench into your search, but I'd look into this as well.

NEW: 3/16/2010: Successful surgery in Brazil w. Dr. Pimenta; Nuvasive NeoDisc at C5/6, and XLIF & ALIF at L4/5/S1 w. PEEK cages. No rods, screws, plates. Non-metal lumbar ADR not available at present time, so went with fusion.
