One piece of hard advice: Even though it sounds like your surgeon is reasonably knowledgable, it will pay you to spend this time while you're deciding to find the most experienced ADR and most experienced fusion surgeon you can find a way to afford (your monetary situation can decide for you if I'm talking about paying yourself or who your insurance will pay). No one promises good results, but as you need the best result you can get, you really don't want to be one of the first few 'trial' patients while the surgeon learns.
Don't take this as advice to put up with a jerk or compromise your recovery by dealing with a surgeon that will not follow through with you, but make sure you are comfortable with who you choose and comfortable with their experience level.
It is an unfortunate reality that often pain can suddenly increase and that can push you to make decisions. If you've laid the groundwork, those decisions are easier to make and don't lead to nagging doubts in the future.
