Default Post op precautions

I just wanted to share 2 important post op recommendations I received after my hybrid (but likely applicable to any implant/spinal surgery). Of course ask your own surgeon, and there are other obvious precautions, but these slip a little under the radar:
1. I was told to have antibiotics before dental cleanings for life. Inform your dentist.
2. My employee health nurse at work said no flu shot until 3 months had passed since my surgery.

Thought it was worth mentioning.

Joell,

That's interesting about the dentist.
Problem is, if I tell them that I read on a forum to ask for
antibiotic, they are just gonna say that's not true and I don't need it.
You know what I mean?
It reminds me of the Dr. Oz show.
All of the stuff Dr. Oz recommends is great if we all had docs like him.
Unfortunately, we don't and most of the stuff Dr. Oz talks about would
never happen with any doc I've been too.

Why did they say to take antibiotic with denistry?
I'm assuming because they have exposed your dental bacteria to your bloodstream but just asking to make sure.

Thanks for posting!

Yes, I'm assuming that's the reason - higher risk of bacteria in the bloodstream through the cleaning process. My husband remembers Mr Boeree saying this in our pre-op discussion (I honestly didn't recall it, but I think I was a little distracted.) I'm surprised you didn't hear the same thing. Might be good to check with him yourself - I'm sure he'll get back to you via email. I know my dentist always asks about any change in medical history. I'll be going in for a cleaning soon, so I'll post what he had to say about it.
I would have never thought about the flu shot. I get one every year since I work in healthcare.

Why no flu shot until after 3 months post-op? I recently had one at a little under 3 months.

my dentist said there's no real data for ADR's, but he gives them anyway. most dentist are very cautious, and would err on the side of caution if they weren't sure.

Mr Boeree returned my email in 2 hours via his PA (that attentiveness deserves its very own post!) And said "Yes, 24 hours of antibiotics for dental cleanings due to the risk of bacteremia". Still waiting on a "why" on the flu shot recommendation at work.

If I remember right there is a thread or post on here about dental work and ADRs, I had a bunch of dental work done before I went for my Adr and the dentist put me on antibiotics because I was going for surgery in a few weeks after I saw him. I am for better safe than sorry

yikes, glad i had my flu shot before... required in my line of work, and i always get it anyway since i'm asthmatic and regularly see pts w/ the flu.

one of my new allergies though, is the flu shot... gotta see an allergist before next flu season to figure out a plan.

still not sure why a vaccination (especially for a virus) would be contra-indicated after any surgery.... am interested to see what he says!

My guess is that going through a major surgery places a certain amount of stress on your whole system when your immune system may be somewhat depressed and any immunization/vaccination on top of that also challenges/stimulates the immune system. They probably just want you healed up well before any more challenges.

I can't seem to find anything concrete in the literature that states why you shouldn't receive a flu vaccine after surgery but the consensus probably varies from doctor to doctor. Being a clinical pharmacist I can't really think of any rational why it would be contraindicated and should be avoided. As a matter of fact every year we make a mass effort to vaccinate all of our Hematology & Oncology patients because they are immunocompromised, and unless they have a true contraindication to the vaccine or an underlying issue that would preclude them from receiving it we hold off. I on the other hand, would be hesitant to use the nasal flu-mist formulation on a post-op patient because it is a live attenuated virus which is thought to predispose one to the disease but in a much milder form. This is open for debate as well.

It would make sense that it is beneficial to receive the flu vaccine unless there is a true contraindication to it. You don't want to be laid up in bed due to influenza at the same time as recovering from surgery. Prevention is the best medicine! If I ever get around to it I will ask both our infectious disease doctor and pharmacist to get their take on the subject. In the meantime I guess I am the guinea pig since I received my flu shot at around 10 weeks due to our hospital policy and right before I went back to work. Every year over 700+ employees in all areas of our hospital are required to be vaccinated due to our line of work.
