You also need a signature with information about your situation. It would be nice if you also gave some history of your situation as it's impossible to comment on one's situation without knowing what has/hasn't been tried, consults you've had, etc. Pictures may be worth a thousand words, but they do not tell the entire story most of the time.

- ADR surgery at L5/S1 on June 23rd 2008 - Prodisc
- Recovery - so far so good!
