I have a similar post for UHC people Anyone approved by UHC? and in this post I am seeking people who have their insuarance companies pay for their ADR. I am starting to gather up my info for my first appeal and my HR department says UHC wants to see "which carriers have covered this procedure and for whom" So if you have had your insurance pay for this, if you would be so kind as to PM or email me your insurance company and I hate to ask, your name as well. Oh and the levels and number of levels would help too as I am a 2 level.
