In need of insurance precedents
 

I'm six days post op with a two level Prodisc C. Feeling really good but now I have the task of fighting Anthem Blue Cross for the reimbursement of the surgery. Part of my appeal is to develop an insurance precedent, which would be a list of those whom have successfully appealed their insurance company or had initial approval. It would be greatly appreciated if you could please PM me the following for my appeal:

Your name
Date of surgery
Type of surgery
Name of hospital
Name of doctor
Name of insurance company

Saw your post looking for precedents back in 2010. I am now looking for precedents. I had a cervical C5/C6 surgery and replacement. Now trying to be reimbursed by Blue Cross/Blue Shield. Any help you can provide would be much appreciated.
