Claim denied a second time
This time they are asking for the Provider to complete a UB04 - CMS 1405 Form. I haven't been able to find this online. The EOB says it MUST be the most current version.

Yes and no. I went direct from the airport to my room at the clinic. Masks worn at all times except when in my room alone. You are not really able to go out to a local shop or anything like that....

Doctors here that tell you adr is jot an option; is because they do not do them and haven't been trained to do them.
Did they even telly you why adr wasn't an option?!
