First car accident 9/03.
Second accident 09/04.
Third accident 09/05.
Herniated L5-S1, discectomy 8/15/11.
Re-herniated 02/12, discogram 11/12. (DDD found.)
Hybrid ADR/TLIF L4-5 & L5-S1 scheduled for 1/2/13.
Been in pain for 5 years, hoping for a solution!
