I think you're doing it backwards... find the doctor that you agree with first, and then that office will perform the necessary authorizations on your behalf.

I did finally find one place here in MN that does ADR. I have set up an appointment for a few weeks from now. I will see what they have to say and go from there.
Do you have a date set yet for your surgery??
