What kind of ADR do they want to use? And where and who is doing the procedure?

Supine MRI shows L4-L5 & L5-S1 have small tears
Nerve pain, muscle pain, joint pain in back and legs
Max pain level: 6/10
Consistant pain level: 3/10
Walking and sitting is the worst
Do I currently need ADR: No but investigating because the pain just keeps getting worse.
