BCBSF Medical Policies (Medical Coverage Guidelines)
Blue Cross Blue Shield Coverage Position above.
See your provider and find the "Coverage Position". It appears to be 1 level in Florida.
The question to ask is if the proposed surgeon in BCBS has done the training and how many ADR/TDR operations have been performed, post-surgery NDI at 6 and 12 months for how many patients.
