I'm ex-military and in my experience, the military docs are notoriously conservative when it comes to pain medications. 5mg every 4-6 hours is probably baseline for pain after a spinal surgery. Your surgery was a multilevel fusion, so I would think you may need something stronger at least in the short term. Let me say again that I am not a doctor, but I am a patient of multiple spinal procedures so my opinion is from personal experience. The ER is your best bet if your surgeon is unwilling to find a way to get your pain under control.
