Looking to to ADR with M6

I'm a new member here and I'm happy that I joined this informative community.
I am 28 years old and I have suffered from lower back pain for about 11 years and I'm committed to fix it this time and get my life back.

-Pain in my L4/5 region for about 11 years since I was 17 years old, my pain came right out of the blue.
- Done Discectomy L4/5 in 2013 but only part of the pain was gone,diagnosed with herniation at L5-S1 at the same time but had no pain at that time in that area.
- I started to feel pain in my L5-S1 in Jul 2015
- I started to feel a sharp pain in my neck C6-7 whenever I was bending my neck forward in Sep 2013
Done ADR L4-5,L5-S1,C6-7 with Dr.Clavel in Sep 2015
