I sympathize with your plight. We generally make the decision to go forward when so dissatisfied either with living in pain all the time or having no life, that living like that is simply unacceptable.
For me, the decision was easy as I'd already had two discectomies and pretty much had no other options... still I second guessed my decision until the very day of my surgery. I think most others feel the same way and freaking out is a normal response.
Regardless of your decision, I wish you luck with both getting your life back and your education.
