Google "FEHB FDA"--you are looking fro a pdf that indicates that all providers of federal employee health benefit plans need to adhere to the FDA approvals and cover the procedures. Now, the only approved use is single level, not sure about adjoining fusions or not, but that's how I would fight. if you cannot find the letter, email me at
