Hi all
I am in the process of my final appeal. If anyone has an approval letter from thier ins company esp if its 2 levels and would share it with me so I can send it in with my appeal would be greatly appreciated. YOu can email me with it and black out any identifying info that you would not want seen by others. I just want to show my ins that other companies are indeed approving the process.

Kim, I don't have an actual approval letter, but I have the Blue Cross statement that shows they did pay on a three level, is that what you need?

Kim,

I have a single level approval letter from Health Net for a Charite. It was approved after my appeal via a specialty match review. If you think it might help you send me a PM with a fax number included and I can get it to you.

I would like to include that statement with my final appeal to BC since they claim FDA approval for a single level only.

Please email or pm me and I'll give me my fax number.

Kim, I"m looking for an authorization by BC/BS of NY but misplaced it.

Trying...it was for a two-level.

Severe, extensive DDD, considered inoperable by Dr. Regan, Lauressen, & some guy at UCLA. Severe foraminal stenosis (guess they can't operate!) and some spinal cord compression that Lauryssen would fix if gets outta hand.

This is Danny C. I just got approval from Aetna for 3 Level Charite. I've since decided to wait for Maverick or Pro Disc to be approved becase of the 3 level situation. If you would like to foward a fax number I would be happy to send you a copy. good Luck
