Hello everyone

My name is Alejandro (38 years old) from Peru. I am glad to be in this ADR Support Community and to know the stories of each one of you in the search to improve your back conditions and therefore your quality of life.

At present I am evaluating to undertake a ADR surgery for my lumbar back at L4/L5 and L5/S1 levels. I am trying to read and learn all the experiences posted by you. By this time, I have sent my spine information to Instituto Clavel in Spain (Dr. Clavel), ONZ (Dr. Bierstedt) and Stenum (Dr. Ritter-Lang) in Germany. I will be gathering all their proposals for ADR surgery in order to take the best option. This process (spine conditiion) has been very difficult for me.

Alejandro, thanks for posting and sharing. It's been a while since I communicated with doctors in South America, but I am sure there are some really good ones in your country and continent! I'll try to find my notes from many years ago.

Es bueno considerar muchas opciones; sin embargo, el Dr. Ritter Lang y el Hospital Stinum han tenido complicaciones graves. Investiga más sobre ellos.

Hi Alejandro. I was wondering if you’ve made any decisions yet. I too am looking at going to Dr. Bierstedt for L4-S1 replacement with ESP disc. Saw some great results posted online, but they don’t tend to show the not so great stories there. Looking for real people with real results, and of course hoping they’re all as good as possible.

I’m a 42 year old male lbp for the past 7 years. In the most recent 3 years I have gone through episodes of acute pain, resulting in having to take meds to subside nerve pain in the legs, being out on my back for 3-5 weeks at a time, barely being able to move.

ONZ. Diagnosis:

Degenerative disc disease (DDD) (dehydrated disc, loss of disc height, herniation) at level L4/5, L5/S1

Hello Dmoney. First of all, sorry for my late response. I am still evaluating which doctor and procedure I will take based on my diagnosis. The first quarter of this year I contacted Dr. Ritter Lang, ONZ Germany and Dr. Clavel. All of them indicated two ADR levels in L4/L5 and L5/S1. However, I also talked to Dr. Ferrandez in Spain and he recommended fusion in the same levels. He told me that ADR will cause destabilization in my lower back and will worse my pain due to my multi-level facet syndrome. This year I have been dealing with back and both knees pain. I would not like to keep getting worse, but I also don't want to have a risky surgery. Certainly, It is hard decision, also considering the different indications received from these doctors. I hope you can take the best treatment for your spine and have the relief that everyone who is going through this is waiting for.

The double fusion that low in the spine could easily lead to problems with your sacroiliac joints and hips. If you have any concerns with those prior to the fusion, I'd be very hesitant about fusing that many levels. Has anyone suggested a hybrid, fuse the level that has the worst facets and use an ADR on the level with the least facet involvement?

Hello Annapurna, many thanks for your message. I am sorry for my late response, but I had covid19 and my recovery process was a bit tough (during weeks). Yes, I have considered a hybrid procedure which can avoid potential problems with my joints (sacroiliac joints, hips and knees). Actually, my knees are hurting a lot, worse than before. However, Dr. Ferrández told me that an ADR solution could generate more instability and according to his view, he would prefer to go through a two-level fusion (L4/L5-L5-S1). I will start again asking other spine doctors abroad.

I don't have the skill to comment on your films or situation with instability but your idea of getting a few other opinions on the subject sounds like the smartest thing you could do. European surgeons are a lot more aggressive, typically, at using hybrid approaches so that will be a known bias you'll have to think around when you get the second opinions but you'll still be able to learn something about why those surgeons say what they say.
