Default How to get FMLA approved?

Hello Everyone,

Can anyone share how to deal with out of work FMLA issues if I go to Europe for ADR? When I had my last surgery in the US my local doctor filled out all the papers, and my insurance covered everything. With ADR, no insurance coverage, and my local doctor is not involved. How do I go about filing for FMLA? Is it even possible?

If it were me, I would have my primary care doctor (PCP) take care of the paperwork. If my PCP refused to do it, he would no longer remain my PCP. He/she should get several office visits pre-op anyway. It's not like he/she is not getting some business out of this.

Your primary care dr. will have to fill it out since your Surgeon is out of the country. If you have a spinal surgeon that will be doing follow up states side they will probably be able to fill it out as well.

Your companies HR if your with a Corporation will have the paperwork as well as Dr. instructions.

It doesn't matter if insurance pays for it or not. You are taking off work to take care of a medical situation that will cause either prolonged absence or multiple absences.
FMLA is federal so it is the same no matter what state you are in. Feel free to PM me if you have any questions. I had to use FMLA 4 separate instances in the past 6 years twice for me and twice for my wife.

Default Fmla

My FMLA, I filled out the notice, phone call really, with HR, then I asked Dr Bierstedt (Malte) to sign this, that they did perform the surgery, and asked them to fax it for me, directly to HR.

When I got back, to Ohio, I visited the primary, my personal doctor, and he gave me an additional 45 days, and went back to work, for another 30 days, half day.

The paperwork should be available from HR, maybe on your company website.
