Default Looking for Doctor Contact Information

I’m looking for contact information for Drs. Rischke, Desai and Bierstedt. I’ve seen the info before on this site, but can’t seem to locate it now. X-rays taken yesterday show I now have bone-on-bone contact at L4/L5, and looking to have surgery done in the next couple of months. Looking for contact info and the process for getting my info to them.

Suffered from back pain (on and off) since my 20's
1994 - Began Chiropractic treatments.
1999 - Added deep tissue message to my treatment regime.
Aug 2015 - X-Ray & MRI show DDD at L3/L4, L4/L5, and L5/S1, with severe herniation and loss of disc height at L4/L5.
Sept 2015 - Two epidural cortisone injections at L4/L5.
Apr 2019 - Updated x-rays & MRI show further deterioration of discs from L3-S1, with bone-on-bone at L4/L5.
June 2019 - Three level ADR scheduled

You are asking for these doctor's contact information, but say in your signature that you have ADR scheduled for June 2019?

Signature is current while his original post was from April... I don't believe there's (necessarily) a conflict.

L4-L5 Broad Diffuse Bulge, mildly contacting left L5 nerve root
L5-S1 Broad Central Disc Protrusion mildly impinging left S1 nerve root
"Mild scoliosis of lumbar spine".
Four central disc protrusions in thoracic spine.

C5-C6, C6-C7 bulging, bone spurs. Imaging not looking good. Successful CP-ESP's placed in Oct 2019 by Dr. Desai.

Delayed lumbar surgery to work on bone density. Considering options now, some but minimal bone density improvement. Will only use LP-ESP.
