I am brand new to this forum. I have a two level need at L4/5 and L5/S1. Are two levels FDA approved and if so, where is the closest doc to do this? I live in Salt Lake City, Utah. Also, my injure is a Fed Worker's Comp, so any info on them paying for this would be appreciated.

Thanks Robin,
I just looked at your posts. It was encouraging to know that OWCP actually approved the ADR. This code business is more than crazy. Today,I called Dr. Bray in Los Angeles, but his office said that they do not take any workers comp. I called Texas Back Institute and they also said that they no longer work with Fed WC. I have been refered to two other docs in LA, who might. I will contact them tomorrow. FYI, Texas Back Institute told me that they could not take my private insurance if it was a WC accident, which it was. I have mailhandlers and have no idea what they would say or if they would approve.....any one know???
