I am willing to pay a fee to anyone who (a)is a resident of NY, and (b) is covered by an Oxford health plan, (c) and had a spinal fusion procedure covered by any of the CPT codes 22612, 22630, 63047, 22842 and/or 22851, (d) subsequent to 1/1/2006, if they will mail me a copy of their EOB for this procedure.
