Default Three or Four Level Disc Replacement

Has anyone here ever gotten their insurance companies to pay for a three or four level disc replacement in the United States? If so how did you do it?

Surgery w/ Dr. Clavel, 3/18/13, M6.
Before surgery: severe spinal stenosis C5/C6 (cord "flattened" per stateside doc), + for Hoffman's & Babinsky's.
At time of surgery: 5 yrs MAX before ending up in wheelchair.
Clavel found L5-S1 partially fused. Had to cut it apart to put in M6.
Please excuse brevity - SEVERE carpel tunnel.

I don't think you can even get a thoracic ADR. The anterior approach is obscured by ribs, sternum, etc. in the way, and at the moment, C6-C7 seems to be as low as anyone will go.

The T-spine is pretty well braced with ribs and each segment has very little motion, so there's not much reason not to have a fusion there.
