What next?
Just received BCBS decision on reimbursement for my surgery in Germany. While they "allowed" the full amount billed they denied reimbursement because "Service provided without authorization." So, I am looking for advice on how best to appeal this decision and would appreciate any insight. Thanks.

Did you call to verify coverage at all with BlueCard WorldWide before surgery?

Yes, in fact, they even sent me the claim form to send in after surgery. Turns out that the "international" division of BCBS just converts the currency to USD and then sends the information to one's home state. I let you know if I get anywhere with appealing...
