Default Anthem Blue Cross of CA

I was informed by Sue Hart that Anthem Blue Cross of California has covered ADR surgery by Dr. Ritter-Lang at Stenum about 50% of the time.

Does anyone have experience in this regard with Anthem Blue Cross of CA? Why do they deny coverage half the time? And cover ADR half the time? Is it just hit or miss bureaucratic inefficiency? Or does it depend on the specifics of the patient? Is it better to seek pre-approval? Or submit after the ADR? How can they maintain such an inconsistent policy?
