Sept 2005 - Developed back pain Nov 2005 - Referred to see Mr Lam. Diagnosed with DDD at L5/S1 with tear. Dec 2005 - Facet joint injections - 6 weeks partial relief Feb 2005 - Follow up consultation. Discussed ADR (possibly around June/July time)
