Anyone have a HealthNet Approval Letter?

Hello everyone. I'm looking for anyone out there that has been approved for a Prodisc procedure by HealthNet (it doesn't matter how many levels you had done.) I specifically am looking to obtain a copy of someone else's Approval Letter, so I can include it in the enormous file that I'm submitting to HealthNet. My husband's two level Prodisc surgery is 2/23/07 in Connecticut. HealthNet suggested to me that I mail this file in now... not to wait to submit it as an appeal if (when) we are denied. My file is up to 111 pages and is growing RAPIDLY, but I would really like to include proof of someone else's approval. (This is something that I see others are regularly doing now.)
